The Grandview CSD #4 Athletic Department will no longer accept paper copies of MSHSAA Sports Physicals. Parents will now use the application PRIVIT to submit all physical information. Parents must set-up a PRIVIT Profile Account and fill out all required online forms. This can be done online at https://grandviewbulldogs-mo.e-ppe.com/index.jspa or on the PRIVIT: Health Profiles App on your smartphone or tablet. After your child visits the doctor, you must upload the completed MSHSAA Physical Form (page signed by the doctor) to your PRIVIT Account.
